Medical card for Domiciliary Care Allowance

You can register your child for a medical card if you receive a Domiciliary Care Allowance (DCA) for that child.

You will need to provide your:

  • PPS number
  • child’s PPS number
  • contact details
  • child's date of birth
  • GP's name and address
Information:

If registering two or more children with different GPs, you must complete two registrations. Select the GP of choice for each child.

Check if your GP is accepting registrations

Electronic GP registrations

If you register your child online and your GP is accepting registrations electronically, we will send your child’s details to them.

If your GP is not accepting registrations electronically, you will receive an email with a copy of your child’s registration details attached.

Print this email attachment, bring it to your GP to sign and then post it to:
