Acute Primary Interface Group

The Sligo General Hospital / Sligo / Leitrim Primary Community and Continuing Care (PCCC)Acute Primary Interface Group is a group comprising Consultant, General Practitioner and Management representatives established to give leadership to and be responsible for the progression of key issues across the acute / pccc interface in the Sligo General Hospital catchment area / Sligo / Leitrim PCCC Services.   

The group aims to:

• use a partnership approach to adopt and put in place a whole systems approach to care delivery to deliver seamless, integrated service responses, in accordance with needs
• adopt a person centred approach to care delivery and work collaboratively to facilitate appropriate care in the appropriate setting
• promote and implement strategies that provide quality patient care and positive experience of services
• ensure that systems are in place to facilitate timely access to / utilisation of services in accordance with needs
• promote opportunities to improve communication focused on the delivery of care along the care continuum
• monitor and analyse activity / key performance indicators as appropriate across the acute hospital / pccc settings
• promote the utilisation of information technology to support information sharing and the delivery of patient care
• promote opportunities for joint learning / networking for those involved in delivering patient care across the care continuum
• continue to promote a culture of and opportunities for acute and pccc service integrated working.

Meetings of the Group

Meetings of the group are scheduled on a bi-monthly basis.

Updates on the group’s activities are provided to the Hospital Management Operational Team, Hospital Management Committee, PCCC Management Team and the Co Sligo General Practitioner Society via the GP representatives.

Current Initiatives

Current Initiatives include provision of data re GP utilisation of Hospital Services, GP RIP Notifications, development of Electronic GP referral and discharge letters, Criteria for Laboratory Tests, Streamlining of GP Postal Services and Annual Clinical Workshop.
