To apply for a Medical Card, you can download and print an application form from this page, fill it in, and return it to your Local Health Office.
The form most people will use is the general Medical Card and GP Visit Card Application Form, MC1.
People aged over 70 whose gross weekly income is under €700 (or €1400 for a couple) should apply using the new shorter Over 70s Medical Card Application Form, MC1a.
All the form are available from the list of pdf documents down the right hand side of this page. Some forms are available in Irish.
You can also collect an application form from your Local Health Office or nearest Health Centre.
The HSE infoline 1850 24 1850 is available 8am - 8pm, Monday - Saturday, and our infoline staff will be happy to answer any questions you have about Medical Cards, post an application form to you, or help you in filling in the form or making your application.
