Children who have been diagnosed with cancer.
The HSE has granted Medical Card eligibility to children under the age of 18 years, who have been diagnosed with cancer. The parent or guardian whose child has had a diagnosis of cancer within the last 5 years, can apply for a Medical Card by completing an Application Form MC1 (B). An Irish language version of the MC1B form (Foirm Iarratais ar Chárta Leighis do Pháistí faoi 18 bliain) is also available
The income details of the child’s parent or guardian do not need to be provided with this application form, but a medical report completed by the child’s GP or medical consultant should accompany the application form.
