The employee selects a bicycle of their choice from a shop on the Office of Public Works (OPW) approved list of suppliers. You can view the list by clicking here. However,as the OPW list is updated on an ongoing basis, it is best to check the most up to date list by clicking here http://www.opw.ie/en/cycle-to-WorkScheme/ .

The supplier invoices the HSE directly and once payment has been processed, the supplier contacts the employee to arrange for collection of the bicycle.

Completed forms should be returned to Joanne Donnelly, Health Promotion Department, HSE Dublin North East, St Brigid's Complex, Kells Rd, Ardee Co Louth.
