Request an EU Digital COVID Certificate of Vaccination for vaccinations received outside of the European Union (EU)

If you are an Irish citizen who has received your COVID-19 vaccinations outside of the EU, you may be eligible to use this form to request a EU Digital COVID Certificate.

Please note that you will not be eligible to use this form if any of the following apply to you:

1. You are not an Irish citizen 2. Your most recent COVID-19 vaccine was received in an EU country or in a non-EU country which has an EU equivalence decision 3. You do not hold an official government issued COVID-19 vaccination certificate from the Non EU country
