Cork Kerry Community Healthcare is committed to providing enhanced care through the creation of 14 Community Healthcare Networks across our two counties.
The roll-out of networks combined with programmes to support older people (The Integrated Care Programme, Older Persons) and people with chronic disease such as asthma, diabetes and others) will move us closer to the aim of delivering the Right Care, in the Right Place, by the Right Team.
This work is putting in place the building blocks for true community care for generations to come, and will allow us to deliver our vision of a healthier Cork Kerry with an accessible, responsive, connected, high quality service valued by all.
