How can I arrange a referral to the EPAU?

If you are unsure if your patient needs a routine EPAU appointment (that week), an urgent EPAU appointment (next day), or A&E review please contact the on call Gynae Registrar.
Who needs an urgent referral to A&E?
Urgent A&E presentations should be reserved for patients who:
Have symptoms of an ectopic pregnancy
- Lateralized pain
- Tender abdomen
- Shoulder tip pain
- At risk history associated with moderate symptoms (tubal surgery, previous ectopic, PID)
Have significant PV loss
- Soaking a pad every hour
- are experiencing a late first or second trimester loss with active pain/bleeding
Are acutely unwell
What can my patient expect on arrival to A&E?
If unstable, Gynaecological Registrar review and urgent management/active resuscitation will be instituted.
If stable, A&E triage and Senior House Officer review takes on average 4 - 5 hours. The Patient will be booked for a routine EPAU clinic appointment.
PLEASE BE ADVISED THAT NO OUT OF HOURS ULTRASOUND SCANS ARE PERFORMED UNLESS AN ECTOPIC PREGNANCY REQUIRING IMMEDIATE MANAGEMENT IS SUSPECTED.
Who should I refer to the EPAU?
Patients with bleeding or pain in early pregnancy (prior to booking visit), > 6/40
Patients with no symptoms, at risk of an ectopic pregnancy, (history of tubal surgery, previous ectopic, PID)
When will my patient receive an appointment?
We are able to accommodate most patients within the week that they present
- If symptoms are slight and LMP suggests a pregnancy < 8 weeks gestation we may advise a longer wait prior to attending the EPAU.
At risk or very symptomatic patients will receive an urgent appointment
What information should I include in my referral letter?
Regularity of period
Use of the OCP within the past year
Parity, mode of deliveries of previous children, previous miscarriages, terminations, or ectopic, previous obstetric complications
History suggestive of high risk of ectopic (tubal surgery, PID, previous ectopic)
IVF/Donor egg pregnancy implantation dates

The EPAU is an appointment only clinic that runs from Monday to Friday, 8:30am - 12:30pm
The clinic is not open on Bank Holidays or Public Holidays.
The EPAU is staffed by a dedicated Gynaecology Consultant/Registrar, Sonographer, and EPAU nurse.

If your patient has very heavy bleeding, significant abdominal pain or feels very unwell they should present to the Emergency Department urgently.
