Sligo General Hospital asks patients to bring their PPSN when they attend

Coming to Sligo General Hospital ?

Please bring your Personal Public Services Number (PPSN)


From today, 1st May 2008,Sligo General Hospital is asking all patients to bring their PPSN with them on their next visit to the hospital. This includes attendances at Out-patient Clinics, Day Services, Radiology and Emergency Departments and planned admissions.

This request is in addition to the existing requirement to bring a hospital number (known as pcn), medical card number and / or details of health insurance membership (if relevant).    

As the PPSN uniquely identifies an individual, it will help the hospital to identify past medical records, if any, which will in turn support ongoing patient care and a safer patient environment.

PPSNscan be found on Social Services Card, Drugs Payment Card, P60 form, P45 form, Tax Assessment, PAYE Notice of Tax Credits, Temporary Payment Card, Pension Book, Pay Slip and on correspondence from a social welfare or tax office.  Alternativelyit can be obtained by contacting the Social Welfare Information Services lo call number on 1890 662244.
