Experience of Transition of Care from Paediatric (child) services to Adult services 

Transition of care is the planned move of a young person’s care from a paediatric (child) service to an adult health care service. The HSE are currently reviewing the care young people receive when moving from paediatric health services to adult health services.

We want to ensure the transition of care meets the needs of young people and their families, carers and friends. To do this we need to understand what it feels like to go through transition of care.

We are inviting young people aged 14 to 26 years who currently are in the process of or have already transitioned from a paediatric to an adult health care service and their family members, carers or friends (18 years or over) to complete a Your Voice Matters questionnaire.
