Towards person-centered integrated care in Ireland
About this project
The National Clinical Director Integrated Care Office is working on a project with the HSE Health Regions Programme and the Department of Health Strategic Workforce Planning Unit to evaluate Integrated Care in Ireland. The project aims to learn from successful practices in Europe and pilot new supports for integrated care.
It focuses on supporting Irelands Integrated Care reform process through the development of a multidisciplinary workforce and improved coordination between hospital and community care.
The project is funded by the European Union (EU) and is being carried out by the Organisation for Economic Co-operation and Development (OECD) in partnership with the European Commission's Reform and Investment Task Force.
