Specialist Geriatric Services Model of Care - Part 1 Acute Service Provision
This initative is aimed at significantly improving access for frail older patients to specialist geriatric teams (doctors, nurses and therapists), day hospitals, specialist inpatients beds and rehabilitation beds. This will improve the quality of care received by older patients and shorten the length of hospital stay.
