Below are details of each Enhanced Community Care (ECC) Integrated Care Hub for “Integrated Care Programme for the Prevention and Management of Chronic Disease (ICPCD)” and “Integrated Care Programme of Older People (ICPOP)” hub within Community Healthcare West (CHW/CHO 2 - Galway/Mayo/Roscommon).

  • Frailty - new or unexpected change in mobility and/or function (e.g. requiring help or dependent for shopping, meal preparation, medications self-management, showering, dressing, walking outside alone etc.)
  • Falls - recurrent or unexplained falls.
  • Cognition Status - new or unexpected change in cognition function.

Also must be:

  • 75 or over.
  • Assessed by referring source.
  • Are aware and agree to referral.
  • Are between 4-6 on the Rockwood Clinical Frailty Scale.
  • Requires assessment and intervention by two or more of multi-disciplinary team.
