When requesting immunisation records please include - Email Address/Postal Address and copy of your Photo ID e.g. passport or drivers licence. Name, DOB, Mothers full name, current address, address at time of vaccination, school attended, hospital at birth and records looking for.
If a request is received before 4pm we aim to post on the next working day. If you were born before 1990 please note it takes approx 24 hours as these are paper records and the archive must be accessed.
