Independent investigation by Mr Justice Frank Clarke

HSE Publishes Terms of Reference for Independent Investigation – University Hospital Limerick

In late December 2023 the CEO of the HSE received a report (Systems Analysis) following the death of teenager Aoife Johnston at the hospital in December 2022.  At the time it was confirmed that there would be an independent and further investigation led by retired Chief Justice Frank Clarke. The HSE has this evening published the Terms of Reference. Independent Investigation by retired Chief Justice, Mr Justice Frank Clarke, into the care of Patient A in University Hospital, Limerick (pdf 7 pages)
