The National Open Disclosure Office facilitates monthly webinars on Open Disclosure related topics. We are delighted to welcome a variety of HSE internal and external guest presenters to these educational events. We welcome all staff to attend the Open Disclosure Webinars.
The purpose of these webinars is to:
- provide continual support to staff across the system in the implementation of the HSE Open Disclosure policy
- maintain communication with Open Disclosure leads, trainers and staff working across our health and social care services, external agencies and patient representative/patient advocacy groups
- keep the importance of Open Disclosure on the agenda across all services
Each webinar is CPD accredited by RCPI and NMBI for those who attend the live event. Recordings are available if you wish to listen back or for those of you who may not be in a position to attend on the day.
We look forward to engaging with you during our webinar season this year.

Presenters
This webinar will be facilitated by the National Open Disclosure Office, QPS Incident Management. We are delighted to welcome our guest speakers:
- Dr Colm Henry Chief Clinical Officer
- Dr Orla Healy, National Clinical Director Quality and Patient Safety
- Bernie O’Reilly, Chair for Patients for Patient Safety Ireland
About the webinar
- the HSE is committed to driving a patient safety culture that is open and transparent. Being truthful when a patient safety incident occurs is critical when caring for patients/service users or their relevant person.
- it has been six years since the HSE Open Disclosure Policy was revised and much learning and insight has amassed over that time. The HSE Open Disclosure Policy 2025 incorporates learning the HSE has gathered in relation to open disclosure including commissioned research, a literature review of international research and best practice, feedback and consultation with patients/service users/relevant persons, patient partners, advocacy services and staff.
- the updated policy also incorporates national developments such as the Patient Safety (Notifiable Incident and Open Disclosure) Act 2023, the Department of Health National Open Disclosure Framework and international developments by the World Health Organisation.
- the revised HSE Open Disclosure Policy 2025 focuses on the importance of support for patients/service users, their relevant person and staff. We must support those impacted by an incident to prevent compounded harm. We must also support our staff, to ensure they are able to deliver on the requirements of the Policy.
- this revised policy will be formally launched by the Chief Clinical Officer of the HSE at this webinar.
