  • Cognition Status - new or unexpected change in cognition function.

Also must be:

  • 75 or over.
  • Assessed by referring source.
  • Are aware and agree to referral.
  • Are between 4-6 on the Rockwood Clinical Frailty Scale.
  • Requires assessment and intervention by two or more of multi-disciplinary team.

Please note that all service users are initially assessed using the comprehensive geriatric assessment (CGA)
