Home Help

Home Support is a partnership between informal (family, friends, neighbours) voluntary and statutory services in meeting a person's goal to remain at home. The service provides assistance with personal activities of daily living and priority is given to all aspects of personal care such as getting in and out of bed, washing and showering, continence care and toileting, dressing and undressing, medication prompt. However your local Health Office is not legally obliged to provide these services.

A Home Support assessment is carried out to identify any personal self care deficits which impact on the persons ability to remain at home. The assessment is carried out by the Public Health Nurse or the Community Nurse. It is done using a standard clinical assessment process as set out nationally.

The allocation of home support hours is based on a standard clinical assessment process based on the level of dependency of each individual together with family and home circumstances. this ensures equity of service to all applicants within the level of resources available. If the demand for Home Support is greater than the resources available you may be placed on a waiting list. This list is prioritised according to clinical risk assessment. Review assessments are routinely carried out to ensure that care needs are being met and resources used as efficiently and effectively as possible.
