Out-of-State Service Arrangement Part I (2026)
This is a standard document for Out-of-State Agencies and is part one of a two-part contract, the other part being the HPSR. Both the Parts I and the HPSR are annual documents.
Disability Composite Template (Out-of-State 2026)
This is a national standard template for use within the Disability sector. It is used to capture data for service specification in Out-of-State Agencies funded by the Health Service Executive that provide disability services.
