Population Health Service Improvement

We work to develop, support and enable a population health service improvement approach to be embedded in clinical design.

A Collaborative Exploration of Population Health Across the HSE National Clinical Programmes

Reports

  • In 2024, the National Health Service Improvement Team collaborated with the HSE National Clinical Director for Integrated Care and HSE Clinical Design and Innovation to undertake a project focused on exploring the alignment of the HSE National Clinical Programmes with a population health approach.

Self-Assessment Tool to Explore Alignment with a Population Health Approach: Version 1 for Testing

Background

Building on our learning from the 2024 collaborative project described above, we have developed a Self-Assessment Tool to Explore Alignment with a Population Health Approach: Version 1 for testing. This self-assessment tool is intended to support those seeking to explore and enhance their current alignment with a population health approach, using the principles developed for the 2024 project.

About the Self-Assessment Tool

The tool provides an opportunity to take a systematic look at how current work aligns with a population health approach, providing insights to help identify gaps and opportunities to build on. It has been developed as a flexible tool to be used by people working in the design and delivery of, for example, clinical programmes, services and other similar areas in the HSE. It is expected that more potential users will be identified as the tool is tested and improved over time, and learning is generated from this process.

Please note: This is the first version of the self-assessment tool for testing. We are dedicated to continuous improvement and learning, and we highly value your feedback on this document. The tool is a dynamic document that will be improved over time through an iterative approach as learning is identified from its use, so that it remains as relevant to users as possible. We are also currently working on a project to co-design a definition and core principles of a population health approach for Ireland (the All Together for Population Health Project), and we will update the tool with the outputs of this project when they are available.

Your feedback provides an opportunity for us to refine this tool, improve it, and further enhance its impact. Please use the Appendix B questions in the tool to guide your feedback following use of the tool and email your feedback to the Project Team at: NationalHealthServiceImprovement@hse.ie  If you plan to adapt or use any content of the tool, please let us know at NationalHealthServiceImprovement@hse.ie, and please attribute any content to National Health Service Improvement, Public Health HSE.
