Neuro-rehabilitation for referrers

The HSE is developing six networks of Neuro-rehabilitation services called Managed Clinical Rehabilitation Networks (MCRNs). 

Each network consists of a;

  • Complex Tertiary Specialist Inpatient Neuro-Rehabilitation – the National Rehabilitation Hospital, Dun Laoghaire, Co Dublin
  • Regional Specialist Inpatient Neuro-Rehabilitation - for example, the Royal Hospital, Donnybrook, Dublin 4 or Peamount Healthcare, Newcastle, Co Dublin
  • Community Neuro-rehabilitation Team (CNRT)- These are specialised multi-professional teams providing a short period of intensive Neuro-Rehabilitation

There are currently two MCRNs in place in Dublin and Midlands and Dublin and South East. In these two MCRNs, the person requiring Neuro-Rehabilitation will be referred into a network of Neuro-Rehabilitation services instead of referral to one service.  The MCRN Network Rehabilitation Coordinator will review the referral and, based on the person’s rehabilitation needs, direct the referral to the most appropriate service within the network. Neuro-rehabilitation network services work closely with existing local rehabilitation services to make the person’s rehabilitation journey as smooth as possible.

Referral process

A healthcare professional in an acute hospital, inpatient rehabilitation or community service can refer to the Managed Clinical Rehabilitation Networks. To refer someone to a network, you must complete the Rehabilitation Prescription.  

The Rehabilitation Prescription (RP) is a word document. It should be completed as a word document. It can also be handwritten, scanned and emailed. Download the RP below.

Once complete, email the RP to the MCRN Network Rehabilitation Coordinator in your area.

Complete the Rehabilitation Prescription with as much information as possible. Incomplete referrals may delay the referral. If a person needs input from the National Rehabilitation Hospital please include their medication list and a consultant’s medical letter. The healthcare professional who emails the completed referral (RP) will be contacted by the Network Rehabilitation Coordinator.

Criteria for referring to a Managed Clinical Rehabilitation Network

The person must  

  • have a home address within that Regional Health Area (link with MCRN Network Rehabilitation Coordinator for details)
  • have a confirmed neurological diagnosis
  • be over 18 years
  • be medically stable
  • have neuro-rehabilitation needs requiring specialist Neuro-Rehabilitation
  • have rehabilitation goals
