Child Optical:

Children up to the age of 12 can avail of child optical scheme. An eye examination must be carried out by a Consultant Ophthalmologist. In order to receive a child optical voucher the prescription along with the applicant’s details is then required to be presented or posted to :

Adult Optical:

The adult Optical Scheme is available to Medical Card Holders over the age of 12 or no longer in Primary School. An application form is required to be completed in full and stamped by an optician of your choice. Application forms are available in most Opticians or at your local Health Centre.
