Make a referral

Healthcare professionals and support workers can refer their patients/service users by completing the referral form by downloading, printing and emailing the PDF version referral form (pdf, 660kb, 1 page) or the word document version IAS-Referral.docx (size 38.3 KB) to iasdublinreferrals@hse.ie

In knowing whether or not a referral is appropriate, it is helpful to do the World Health Organisation’s AUDIT tool (pdf, 563kb, 4 pages)  (Alcohol Use Disorder Identification Test) with your patient/service user, or using the online version on the HSE’s website. Scores of over 14 warrant, with the person’s consent, a referral into the Integrated Alcohol Service.
