National Framework Documents

The National Framework for the Integrated Prevention and Management of Chronic Disease builds on existing policies while also describing a continuum of health promotion, disease prevention, diagnosis, treatment, disease management and rehabilitation services that are coordinated across different healthcare providers and healthcare
settings.

The 10-step guide to support local implementation is an accompanying document to the National Framework. It describes the 10 key steps
to enable the implementation, embedding, monitoring and evaluation of an integrated model of care for the prevention and management of chronic disease at both the national and regional levels.

The Integrated Model of Care for the Prevention and Management of Chronic Disease is at the heart of the ‘National Framework for the Integrated Prevention and Management of Chronic Disease’ and demonstrates how “end-to-end” care can be provided within the Irish health services.

Documents relating to the other chronic disease clinical programmes are available to read or download on their individual programme web pages
