The National Medication Safety Improvement Programme, “Safermeds”, is one of the priority safety programmes within the HSE Quality Improvement Division. We will work with patients, healthcare professionals and organisations to reduce patient harm associated with medicines or their omission. The programme applies the Framework for Improving Quality.

Medication Record Templates for Adult Acute Hospitals

We are pleased to share the Medication Record Templates for Adult Acute Care with you. 

The templates build on the extensive previous work led by Mr Tim Delaney under the guidance of the National Medication Safety Advisory Group, with national consultation and testing in Portiuncula, Naas, Beaumont and Cork University hospitals. We have now updated and redesigned the components of the Record and are making them available as templates to aid the development or revision of local Medication Records.

Preventing VTE in Hospitals Improvement Collaborative

Hospital-acquired Venous ThromboEmbolism (VTE, blood clots) is the most common preventable cause of in-hospital death.  Assessing patients’ risk of VTE and bleeding and choosing the appropriate thromboprophylaxis (e.g. medicines and/or compression stockings) for them early in their hospital admission reduces their risk of VTE.  The HSE Quality Improvement Division is working with multidisciplinary teams from 29 adult acute public and voluntary hospitals to achieve appropriate thromboprophylaxis for their hospital’s in-patients, to reduce the risk of blood clots (venous thromboembolism, VTE) and to minimise harm and waste associated with unnecessary thromboprophylaxis.

This, the first national quality improvement collaborative, commenced with a learning session on 16th September 2016 and will run until September 2017.

Medication Safety Intensive Course

In October 2015, the Irish Medication Safety Network (IMSN), Rotunda Hospital and the HSE Quality Improvement Division hosted over 100 participants at the Institute for Safe Medication Practices (ISMP) Medication Safety Intensive course. Over 200 healthcare professionals also attended the medication safety summit. More information about the course and summit are in the link below.

Consultation: Priorities for Medication Safety
